Unite Connecticut is a coordinated care network of health and social service providers. Partners in the network are connected through Unite Us‘ shared technology platform, which enables them to send and receive electronic referrals, address people’s social needs, and improve health across communities. Unite Connecticut is sponsored by the Connecticut Hospital Association, Yale New Haven Health, and Community Health Network of Connecticut. The network is supported by a Connecticut-based Unite Us team focused on community engagement, network health and optimization, and customer success.
Joining the network is at no cost for community-based organizations and many organizations that are considered part of the safety net, like community health centers, tribal clinics, and mental health centers.
Unite Connecticut is a coordinated approach, connecting hospital-based care providers and community-based social service organizations through a shared technology platform powered by Unite Us. Whether it’s housing instability, lack of access to food or transportation, unemployment, experience of violence or other conditions, healthcare providers can screen and directly refer patients for services.
This close interconnection ensures appropriate follow through and minimizes opportunities for patients to fall through the cracks. The first such system to be convened by a state hospital association, it has gained strength and participants. Nearly half of the state’s hospitals are either live or preparing to onboard and more than 220 community-based organizations are active members in the network. Participating hospitals include Yale New Haven Health, Griffin Health, Waterbury Health and Nuvance Health.
